What is Dry Needling?

Trigger point dry needling is a physical therapy technique that decreases pain in your muscles. Pivot Physical Therapy’s trained physical therapists adhere to a western medicine approach and are not performing acupuncture. Our goal, as musculoskeletal specialists, is to restore normal muscle and joint function. The dry needling technique is used in conjunction with a comprehensive evaluation that complements other treatment techniques to decrease pain and restore function.  

Dry needling physical therapy uses a solid filament needle to treat muscle trigger points. These points are a highly localized, hyper-irritable spot in a taught band of skeletal muscle fibers. These muscle trigger points play a role in producing and maintaining the pain cycle. They can alter muscle performance, as well as generate pain along common referral patterns.  These points develop in muscle for various reasons, including referred or local pain, inflammation, athletic injury or other causes. 

How Does Dry Needling work?

The mechanical stimulation of the muscle produces a local twitch response or rapid depolarization of muscle fibers. After this process, the muscle activity dramatically reduces, which results in relaxation and decreased pain and dysfunction.  This decrease in pain is related to the removal of muscular compression on your joint, nerve, and vascular tissue. Sometimes, the insertion of the needle also reproduces “referred pain” symptoms.  This is a positive sign confirming the trigger point as the source of the pain. Dry needling is highly effective for chronic muscle and repetitive sport injuries.  

Acupuncture vs. Dry Needling

What is the difference between dry needling physical therapy and acupuncture? The dry needling technique uses the same tool that acupuncture uses, but with a different theoretical purpose. Traditional Chinese medicine practitioners use the holistic treatment of acupuncture to normalize energy, or Chi, in the body to cure syndromes.  Although an acupuncture needle is used in dry needling, dry needling is based on traditional reasoning of Western medicine. The sites for the needle insertion are located in specific myofascial trigger points in skeletal muscle, which can differ from acupuncture.

How is Dry Needling Different from Trigger Point Injection?

Trigger point dry needling does not deliver any medication.  Therefore, the treatments can be done more frequently with no adverse side effects.  Traditional trigger point injections use a hollow, hypodermic needle to inject substances such as saline, Botox or corticosteroids. The theory suggests that the “needling effect” is the most important part of the process rather than the chemicals injected.  This does not mean that certain patients will not have greater indication or benefits with injections rather than dry needling and vice-versa.

Physical Therapy for Cancer Patients

Pivot Physical Therapy provides lymphedema care at multiple clinics. In fact, our North Carolina clinics make up the largest and most comprehensive Lymphedema Treatment Centers in central North Carolina. Our certified therapists specialize in the treatment of lymphedema caused by cancer-related surgeries and treatment or vascular insufficiencies. Our therapists take a comprehensive approach to lymphedema therapy and address the underlying source of swelling.

What is Lymphedema?

Lymphedema is the swelling of soft tissues as a result of the accumulation of protein rich fluid in the extra cellular spaces. It occurs most frequently in the arm and leg but can also be seen in the head, neck, chest, abdomen and genitalia. Lymphedema can be greatly reduced and managed through Manual Lymphatic Drainage (MLD) and Combined Decongestive Therapy (CDT). This includes compression bandaging, skin care, and patient-specific physical therapy exercises. Most patients will see significant results within the first two weeks of therapy.

Symptoms of Lymphedema

If you are experiencing any of the below symptoms, you might have lymphedema. Remember, early detection is key to preventing and minimizing the effects of lymphedema.

Lower Extremity

  • Swelling in the foot or ankle
  • Shoes become tight
  • Tight skin or swelling in the base of the toes
  • Numbness and tingling in the feet/toes
  • Hardness, fullness, or heaviness in the leg
  • Feeling of heat in the leg or affected side

Upper Extremity

  • Fullness or hardness of breast post lumpectomy
  • Rubbing of the upper arm on the chest that did not exist prior to surgery
  • Bra begins to feel too tight
  • Fullness, pressure, heaviness in the arm
  • Rings or jewelry feel too tight
  • Feeling of heat in the arm or affected side

What is our Functional Movement Screening Program?

The functional movement screening consists of 7 fundamental movements used to assess an individual’s ability to move efficiently without limitation. Limited mobility at any given joint can lead to compensations, altering an individual's movement pattern and placing him or her at increased risk of injury.

The FMS program at Pivot Physical Therapy utilizes the results of the screening to address limitations through corrective exercises

A Comprehensive Approach

Our Pivot Sports Medicine team understands every concussion is significant, unique and potentially complex. The mechanism of injury that causes a concussion can damage multiple areas of the body, as well as the brain, including the cervical spine, the inner ear and the eye. Associated damage to multiple areas can cause overlapping signs and symptoms that if not accurately diagnosed and treated, may delay recovery.

Our trained physical therapists and athletic trainers are an integral part of a sports medicine team approach to evaluating, treating and managing patients who have suffered from concussions and concussion related problems. We communicate directly with physicians and other healthcare providers in a continuum of care model to ensure that appropriate care and rehabilitation is provided for an athletes’ safe return to play.

Every patient is carefully evaluated, screened, treated and monitored until they are fully recovered. An individualized rehabilitation program is designed and implemented by our trained staff to meet the specific needs identified in the evaluation and diagnosis. Each program has safe, structured progressions.

Occupational therapy assists with the rehabilitation of getting back to the job of living life.  OT programs use functional, daily tasks (or occupations) to help patients return to important daily responsibilities like preparing a meal, managing a household, performing a vocation or playing with children.

Hand Therapy

Our certified hand therapists have been licensed practitioners for a minimum of 5 years, have obtained over 4,000 hours of direct practice treating hand injuries and passed a comprehensive exam including clinical skills and extensive knowledge of anatomy and physiology of the hand, wrist and elbow, or the upper quadrant.

Hand therapy can be a treatment option for any age group.

Injuries often treated by a CHT in an outpatient rehab clinic include:

Is your Balance Problem an Ear Problem?

At Pivot Physical Therapy we have certified vestibular therapy clinicians that help patients recover from inner ear discrepancies due to injury or illness before it causes a loss of equilibrium, resulting in dizziness, vertigo, loss of balance and other unpleasant symptoms.

What is Vestibular Therapy?

Vestibular Therapy refers to treatments carried out by a specially trained physical or occupational therapist to improve the function of the inner ear following injury or illness.  After injury, the signals that our inner ear sends to our brain are not always accurate, causing disconnect between what our eyes see and what our inner ear tells our brains is happening.

Benign paroxysmal positional vertigo (BPPV) is a common cause of inner eat problems. BPPV is the result of tiny calcium carbonate crystals displacing from their normal location in the utricle of the inner ear due to infection, trauma or aging

Infection, injury and disease can cause long-term damage to the structures of the inner ear, which can lead to over-dependence on the visual system (eyes) and the proprioceptive (muscles and joints) for information on the body’s equilibrium.  Such long-term symptoms can lead to behaviors or postures that limit functional activity or head movements, which detracts from quality of life.

Functional Capacity Evaluations

What is it? The Functional Capacity Evaluation (FCE) is a comprehensive assessment of a client's overall functional abilities. The evaluation measures range of motion, strength, endurance and tolerance to work-related tasks.

What does it do? This comprehensive assessment is an effective tool that allows us to determine your ability to fulfill your required job duties, whether it be typing on a keyboard or lifting heavy boxes.

How does this assist my work rehab? The data gathered from the FCE also allows us to establish a baseline of your functional level and compare it to your specific job description. We can then develop an effective therapy program, a timeline for your safe return to work and parameters to prevent re-injury.

Return to Work Evaluations

The Return to Work Evaluation is a modified Functional Capacity Evaluation. It is shorter in length and streamlined to the patient's specific job description and requirements. The information obtained is used to determine if the patient is physically able to return to work and perform his or her job safely and without complication.

Post-Offer Screen

The post-offer screen evaluation is designed with input from the employer to determine if a candidate for employment is physically able to perform the requirements of the job. This information allows the employer to place candidates in the most physically appropriate job while minimizing the potential for injury.

The Healing Power of Water

Pivot Physical Therapy offers aquatic therapy services as part of your rehabilitation and overall wellness. Our physical therapy and occupational therapy staff is fully trained in aquatic rehabilitation and will design an aquatic program and aquatic therapy exercises based on your unique situation, needs and goals.

About Aquatic Physical Therapy

Aquatic physical therapy is performed by a licensed physical therapist or occupational therapist. We use the physical properties of water to assist in patient healing and exercise performance. These properties include buoyancy, which allows less weight bearing through the joints, and viscosity, or resistance, which is patient controlled by the speed of the exercises. Pressure from the water assists in promoting circulation and decreasing swelling. During therapy, the water is kept at a comfortable temperature of 92-94 degrees.
